I hereby declare, under penalty of perjury and in good faith, that all information, documents, statements, and representations submitted during this application and screening process are true, accurate, complete, and not misleading.

I expressly acknowledge and consent that all identity verification, background investigations, criminal screening, compliance reviews, and due diligence procedures associated with my application shall be conducted in accordance with the policies, screening methodologies, compliance protocols, and applicable regulatory standards of Checkr, Inc., together with any applicable domestic or international compliance requirements.

I further understand and expressly accept that any material omission, false declaration, misrepresentation, undisclosed marital status, undisclosed criminal history, identity discrepancy, falsified documentation, or any adverse finding identified during the screening process may result in the immediate termination, suspension, or permanent disqualification of my application, at the sole discretion of Zafira Match Agency.

I expressly understand, acknowledge, and accept that the $400 USD retainer constitutes a non-refundable participation rights fee exclusively associated with the evaluation, screening, administrative review, compliance verification, and candidate consideration process.

This retainer does not constitute, in whole or in part, payment toward any social event, private introduction, matchmaking event, travel arrangement, membership benefit, or future service offered by Zafira Match Agency, and under no circumstances shall such amount be credited, transferred, applied, or refunded.

In the event of disqualification, withdrawal, cancellation, incomplete documentation, non-compliance, adverse screening results, or false representations, no refund, reimbursement, credit, or compensation of any kind shall apply. Any future participation shall require the initiation of a new application from Step 1, subject to a new retainer payment.
